What you need is as easy as ABC, 123!
- Complete Learn & Leap application for services.
- Get a copy of the most recent Autism diagnostic report. Don’t have one, give us a call & we’ll guide you to initiating that process with your doctor!
- A copy of your insurance card.
- FINAL STEP – send them on over to us at [email protected], Fax: 919.377.2395, or swing by to personally drop them off and check out the center between 3-5pm.
I’ve submitted my application, now what:
Step 1
Once you submit your application we bring you and your child in for a family interview! We get to know you and you get to know us to best determine if we are a good match for each other. We believe that this step is crucial in determining care for your child. It is critical that providers and family are a great match to ensure effective care and treatment for however long your child may need it!
Step 2
If all parties believe we’re a great fit – we will contact your insurance to get an authorization to begin an initial assessment. This initial assessment will be scheduled with you and should last no more than 2 hours. Please note parents will need to be present for at least 1 hour of this time.
Step 3
After the initial assessment, the BCBA will compose an initial treatment plan personally curated for your child based on their results! This treatment will include the recommended hours for ABA therapy. Once this plan is created, you will get a copy to review, approve and sign.
Step 4
Once all parties have reviewed, signed and approved the treatment plan, your BCBA will submit the request for treatment to your insurance provider for final approval and authorization. Once this is approved….
Step 5
We start services!!
This may sound like several steps in the process but our team will be with you every step of the way to ensure your child receives the appropriate medical necessary services provided, and your family is fully supported!
